States are increasingly seeking to leverage Medicaid to provide health-related social needs (HRSN) services and supports, particularly through section 1115 demonstration projects. Pregnant and postpartum individuals and children are key populations states are targeting in their efforts to cover HRSN services. Addressing unmet HRSN among these populations can help stabilize coverage, improve access to care, and complement traditional medical care to promote maternal and child health more broadly.

Though states have several pathways available to cover HRSN services, our analyses focus on states’ use of Medicaid section 1115 demonstrations. The charts below provide details on pending and approved state section 1115 demonstration proposals to cover housing, nutrition, or other HRSN services and supports – the supports identified only include those that contribute to direct provision of services (e.g., does not include case management, education, or assistance activities) and as a result may not be a comprehensive list.

Under CMS guidance, eligibility for HRSN supports must be medically appropriate and based on clinical and social risk factor criteria. Potential eligibility criteria for pregnant and postpartum individuals and young children to receive offered HRSN services are identified in the first two charts.
